Because there is only a thin cloth covering, the mattress does breathe. However, if I were to go with a foam mattress, I'd definitely go with a Froli system or maybe roll or two of HyperVent. This way, air circulates under the mattress, so one doesn't get mildew underneath.
I wound up building an elevated (2") platform of 1x2s spaced 6" on center.That allows air flow between the mattress and the base.
