Trailer wheel lug nuts should be checked (re-torqued) after a wheel has been reinstalled at 50 miles, at 100 miles and again at 200 miles. Lots of torsion/strain placed on trailer wheels/tires when maneuvering....especially when backing into a spot.
When torquing.....one click is all that is necessary.
If 'over torqued' (or installed using impact wrench) stud can be stretched/threads weaken leading to failure.
Wheel was either overloaded or not properly torqued
