Were any cross threaded? Had a similar situation but with my previous commuter car. Tire place cross threaded a couple of lugs causing extra stress on the other 3 (the 2 cross threaded ones weren't completely against the wheel). Sheared one off. Luckily I didn't have a wheel fly off, but I could see it happening, even if the bolts felt like they were torqued. I could easily see a factory cross threading a lug and without the offending lugs, you may never know.
