Try online companies and compare pricing. (Keep the foam base if possible.) You can send them your specs or mail an old cushion cover as a sample pattern.
Unfortunately, both fabric and labor are pricey. No point using a cheap fabric that won't hold up. Sunbrella or similar will resist fading and rotting.
