Not sure how that could happen-- would need exactly where you read what:
If each outer is hot (as it should be) AND either the center round (ground) or center straight (neutral) was hot, then CHECKING BETWEEN EITHER HOT AND WHICHEVER OF THE CENTERS WAS HOT WOULD EITHER READ ZERO VAC OR 240 VAC.
