I would have to say that anything "Can" happen. When I plugged into a 30 amp reverse polarity plug with my RV, the Trace inverter/charger did not like it - to the tune of $450 in repair costs!
If the reverse polarity lights come on when there is power between neutral and ground, then that is the problem.
Wires underground can melt together, and cause the wires to change polarity, rare but it could happen. Tree roots might be the cause too.
