Sounds like a loose wire somewhere or a bad switch. When it does work, everything seems fine... I'd pull the cover and start wigglin wires while it's running to see if you can reproduce the problem. Of course you need to be familiar with electricity so you dont get shocked and not stick your hand in the fan.
