Had a similar issue once. At a seasonal lot we had a for a few years a friend came over. He told me they were having word things happening with their electricity. They knew I am an electrician, so asked for my help. I had my meter and went over to their place.
After some figuring I found that hot was connected to ground terminal, neutral was connected to hot terminal, ground was connected neutral terminal. It was almost as if all the wires were shifted out of position by one terminal. This was on a 15A cord. What helped me figure it out was when they told me that after the cord end was replaced, was when the trouble started.
