My guess is that the filter caps are discharging and the converter is being powered back on before the thermistor has had a chance to cool sufficiently (and restore its inrush-limiting function). If this happens often, then the stress of the repeated surges might be causing the thermistor to fail. The bleeder resistor across the caps might also be of such a low value (possibly to meet UL approval) that it is discharging the caps much too fast.
