It seems it has only blown up on start-up, but also only when that start up is too soon (where too soon is defined as when it blows up!) after a shut down. (such as restart soon after the Honda pops a breaker from some other load being added)
I don't think it ever had trouble just from a regular first start, a charging run, then a shut down till next recharge in a few days.
It is possible that there is an accumulative effect, where after a number of such no problem starts, it will fail on the next too soon one. (where maybe you can get away with a too soon one when it is new)
I don't have proper data on all that, just the impression I have as to what has been happening. Best I can do IMO is just replace that with an 020 and see if it happens again with me being more careful on start-up procedure, especially avoiding a too soon event.
