Usually vaccines use killed viruses.
And yes, if you give anything, even pepsi, to millions of people, someone is going to have a bad reaction. But, before vaccines, 3 out of 5 children died from smallpox alone. The diseases kill far, far more people than bad reactions to vaccines. That is the point.
Nothing is perfectly safe, but some things are more dangerous than others. It all depends on how likely you are to run into hepatitis. If it's highly unlikely, then your best choice is to skip the vaccine. But if you are traveling in certain parts of the world (or working at the sewer plant), the smart move is to get it.
