Couldn't the incorrect wiring possibly be in the adapter? From pictures it looks to me like the neutral on the L5-30 is the narrower of the 2 power blades. That could make for an easy bad assumption when wiring either the generator or the adapter. Easy enough to check, just take the voltage readings at the twistlock instead of at the adapter.
