Not good. Curious - why is that a residential type recept. and not an SCD RV type? Someone changed it? The melted wire can't have been terminated properly.

No there aren't. The one in our bathroom has a surface mounting outlet box to get enough depth along with a standard GFCI recept. The reason there isn't one is that the GFCI circuitry takes up too much space and there'd be no room for the IDC terminals.
FWIW, here is Hubbell's installation brochure for their
Wirecon SCDs. Shows pushing the wires in with channel lock pliers along with using the back cover to push the wire in.
