Codes usually only apply to a new installation that is required to be inspected by a municipality or "Authority having jurisdiction".
Once the system is in place it most likely will never be inspected again and the older the installation the more likely it was never inspected to begin with.
I know of a PA state park that has 30 amp services installed in the early 80's that has issues when the occupancy is high with todays hig demand rigs and people dog boning 50 amp rigs. They have had many complaints of fried equipment. The solution was to call them 20 amp sites on the reservation site and in brochures. That gives them an out when stuff goes poof despite it still being a 30 amp receptacle on a 30 amp breaker.
That is the same state who oversees all the inspection entities for the local municipalties. Do as I say, not as I do.
