Using a bigger hammer is a must, using a small hammer just peens them to death.
If you leave the bolt sticking out past the hole and end up mushrooming it you have to cut it off flush and punch it out.
Using heat, penetrating fluids, twisting it and pounding to drive it out are all good ideas and can work together to get it out.
At a machine shop I worked at, if we had an expensive piece that we had remove a stuck bolt from we sent it to a company that used sound waves to loosen the bolt. That was in the mid 60's.
