With reasons why to never do what you did , connect to one power while the other one is hot
You found out why
The lock out failed, the contacts stuck?
There was a cross connection between two different power sources
Utility power will win every time
The generator might also be damaged
I do not think it is permanently wired connected or there is any mis wiring, or the generator would be damaged by shore power even if not running
The first time shore power was connected
The ts is electro mechanical, and mechanical can fail
