I think the primary cause of regulator failure is caused by the propane supplier overfilling the bottle.
When over the listed 80% there is a very good chance of liquid hitting the regulator when the valve is opened.
I keep a short hose (2ft) with the old POL style fitting. The odd time I get back a bottle that seems heavy I use this hose to make sure no liquid comes out when the valve is cracked. And yes it has happened a couple of times, don't always trust the OPD valve
