You misunderstand... they aren't concerned that you are leaving without something you paid for, their worry is that you are leaving with something the cashier accidentally failed to scan, and so you didn't pay for it.
I've been a Sams member for years, usually have a flatbed full of large items, and have had this happen to me a couple of times. They never thought I was trying to steal anything, they knew exactly what happened. Their cashiers miss stuff on occasion, because not all of it goes down the belt.
In all the years I've been going to stores that do this, I have seen multiple cases where the cashier forgot to ring something up. I have yet to see someone slip an extra item into their cart.
Years ago when I worked retail, we were told to always remember BOB. Bottom Of Basket - because cashiers were repeatedly missing the items underneath.
