Torklift - Yep, pricey, but the best way to do it.
I have the same setup and use the 24" extension with my 2009 Silverado Dually, Arctic Fox 811 and a 20' enclosed car hauler trailer.... I've probably put 5000 miles on the truck with that trailer, and many thousand with a smaller trailer I sold a few months ago.
