Can anybody tell me what make/model this is?

Thanks Peg Leg :-) My husband can fix pretty much anything. I'll do the cosmetic stuff & he'll work on the problems. He does AC & plumbing also so I'm not looking for something pristine at all. I'm just trying to find something that isn't completely rotted from head to toe lol!
