Get 1 or 2 good recovery straps and tie that side of the axle to the frame. Take wheel off. Drive slow and easy and you should be able to make it to the repair facility. Stay off the brakes as much as possible.
X2, I also have left the trailer hooked to my truck and backed it onto the tilted flatbed truck just enough to get the jack down on the flatbed. I have never tried but I don't think the tongue jack would be sturdy enough to move the trailer on.
