That's the trick. There are two kinds of forms. Finding the correct one took me a lot of web searches. I can't remember now what the magic selections are on the FCC site, but there are tutorials that help you do this on the web. The complex forms are intended for commercial licenses. I can tell you that I found the right form in the most unlikely place. The FCC could certainly make locating easier.
