Did the 7.3's still have an EBPV (Exhaust back-pressure valve) in '03? That sounds exactly like what my '95 used to do when the BPV would stick closed. The best thing I did on mine was remove the EBPV entirely and install a USGear exhaust brake. The exhaust brake can be used to help the engine warm up on cold mornings. Plus, you'll love having the exhaust brake when you're going down a steep grade.
