The exchanges I've seen are around $20 for a 5 gallon tank. Refills are about $13. I really don't see how these exchanges are making much keeping their prices that high.
As mentioned, the exchange tanks are not full, but filled to about 75%. Plus, I would think many find it more convenient to just exchange a cylinder when they go grocery shopping (or wherever it may be) than make a special trip or extra stop to have it refilled somewhere else.
