My neighbour used one from new to somewhere over 300,000 miles. His was a company truck, an '02, had the 7.3, manual transmission and was in an F 350 dually, 4WD with a utility box. He was a lineman for the local utility.
He said it was very reliable, very little ever went wrong with it and many times it was towing those hd trailers with huge cable spools. It was used very hard...it was a working truck.
Our climate is extreme...can go to -35-40 in the winter...up to around 90-95 in the summer, lots of snow and ice for 5 to 5.5 months of the year. Lot's of rough roads up here. I would say my neighbour's 7.3 was operated in severe service conditions.
The 7.3 is an International engine, with few mods made by Ford, to put out more hp than it could stand. I think the relatively moderate hp/torque state of tune (but the 7.3 still had more than enough power) contributed to it's durability.
I can only speak about my neighbour's long term experience, but he was very happy with this 7.3 powered F 350.
