Hello, my name is Doug and I have a problem...

I obviously needed a checklist during this process, because I forgot to obtain some of the information listed in this thread. In my defense, if there is one, this encounter took place after I had been driving for the past 6 hours through 6 states, so I may have been slightly road-weary at that point. :)
