We had OnStar on two vehicles. The first one was totalled and automatically coordinated EMS response for my wife. Then we had it on the Acadia, which we never touched.
IIRC the system uses the cell towers, but have higher power transmitters for better range. It can still be defeated by terrain though.
I imagine if you cut the cable to the antenna it would be effectively disabled though.
