our sons work trailer had a similar thing happen after the motor pool fixed a flat on that tire. Motor pool concluded after examining the hub that they were not tightened to the proper torque (loose).
right or wrong?? who knows.
but remember, most trailer rims/hubs are lug centric and the lugs take the all the load. so if they start to get the least bit loose, they get hammered every revolution. Tow vehicles are almost 100% hub centric rims, and the hub ring takes ALL the vertical load, the lugs just hold it in place. Even a slightly loose lug will only take lateral load and rotational change load, the hub still takes all the vertical load.
So loose or over tight lug nuts are a bigger issue than on a car/truck
