Only your local DMV office knows for sure.
Here in Montana, when I had my old dually, no DOT inspection required, no commercial license required (for me or the truck), and no medical card. The truck was registered for 14,000 GVW. The license plates were PERMANENT because the truck was more than 11 years old.
I know, that information is totally useless to anyone who does not live in Montana. ONLY your DMV officials can give you the information you require, for YOUR state!
