Good grief, we don't change our fuel filter that often, every 10,000 miles and some only every 15,000 miles. Ours has 101,200 miles on it, it is an 03 and we recently had an injector go out on a long trip. Replaced all 4 on the left side and are getting ready to do the right side now, because we're afraid to take another long trip without doing it. Several people on here have said, it's the fuel that burns dirty and sticks to the end of the injectors and causes them to stick open or shut. Ours stuck shut. There is a diesel additive you can buy and put in the fuel tank, called Diesel Extreme that is supposed to clean the injectors. It's made by Hot Shot. We haven't tried it yet, so don't know how it works. Someone else will probably have a different perspective on this.
