http://auto.howstuffworks.com/question432.htm describes what a sway bar does better than I can. A sway bar connects to the frame which I believe has the net effect of a stronger frame since the frame can't move as much with the bar that ties both sides together.
At the end of the day, people here report bigger stronger sway bars help. How it works is probably secondary.
