Don't know how big a camper you want to go, but just about every RV manufacturer now makes the models with the outside kitchen, usually a big door that opens up and exposes the kitchen from the door side, not the back. Maybe this is something you can look into.
My Outback has a flip down stove and sink outside. No refrigerator, but that permits all cooking outside. Not the best photo, but you can see the flip down stove-sink under that middle window, right beside the slide out. We love ours. We very seldom, if ever cook inside:
