Some have been "getting away with it", most have been using available well-engineered equipment to get the job done. I tend to throw in with the latter group. Not sure who you think is "dead set" against extending the hitch or towbar, but count me among those who likes facts and figures to back up my "guesses" and "opinions". There are formulas and measurements that verify what is going on behind the receiver and there are "hunches". It's everyone's choice as to which they believe.
FYI: I run an extended shank when towing my 5 X 8 cargo trailer as I have plenty of capacity to do so. I did the math.
