I've owned and towed heavy with both...My 13' had 37,000 trouble free miles on it and it towed 16,300# flawlessly in the mountains. This truck we have now, with 3,000 miles on it tows my heavier Jayco TH even easier, but it has 50 more ft lb of TQ, and yes, I can tell it. The 68 RFE also worked flawlessly.. I paid the extra money for a medium duty tranny strapped to a medium duty Cummins diesel for our towing purposes when we retire end of July. If you have the money, it's WELL worth it in my opinion, based on fact, not hear say..I know what mine did and how it ran, and the 16' is even smoother, and much more stout..LOVE IT!! Glad I paid the little extra for it...worth every penny..

Keep in mind the comments bellow are coming from a Cummins/Aisin owner:
I think there is some chest beating about the Cummins and Aisin being medium duty. This is old news to the two other manufacturers that have run the Duramax/Allison or Powerstroke/Torqshift for years in their Class 4 and 5 offerings. Now Ford is even confident enough to use their combination in Class 6 and 7 vehicles.
I have to agree the 68RFE is probably sufficient for any current SRW application, but I would question using it in a DRW. Only time will tell whether both of these transmissions stand up to heavy use, but Ram wouldn't outsource the transmission to Aisin if it could build something comparable itself.
The horsepower/torque rating competition is getting ridiculous whether on the diesel or gasoline side and you really need to look at the Class 4 and larger offerings to get realistic ratings. Here you can see which engine and transmission will consistently output the most power and tow or haul the most weight. I don't think you should be getting the Aisin to chase a slightly higher engine tune, but look at the Aisin if you are towing close to the capacities of 68RFE (which usually means DRW applications).
