do these little battery operated or 12 volt fans that go inside of frig really help with keeping things cooler

We bought a Camco fan and have used it on the last two trips we've been on. It does help move air to the bottom of your refrigerator you just have to leave room for the air to circulate. This fan seemed to have better reviews than the others I looked at the only negative thing is it doesn't have a on and off switch you have to remove the batteries which really isn't a big deal.
