This is the response I got after relaying the info from tech you emailed to the one I've been working with:

The voltage to the fan should be between 10.8 and 13.2 volts, but it can handle the surge of up to `16 volts for a bit, but not a continued voltage of above 13.2
I am not sure if that was miscommunicated by Dale, or misunderstood by the poster, but the constant voltage to the fan should be 12 volt plus or minus 10 percent. This might change in the future as new boards are being looked at, but right now it should be in that voltage range.

This appears to be an issue that isn't going to be resolved with a new PC board. I ordered two of the buck regulators, one for my fan, and the other for a future use. If I still have the issue after installing the regulator, then I'll likely need another replacement board.
