When I was a truck driver what used to scare me the most is when the road was re-paved (added couple inches of asphalt) and the sign was never updated or changed.
I had that happen once:
Flatbed truck loaded with 288 ~60 lb bales of straw, 13'4" high.
Viaduct in Urbana IL marked 13'6".
No problem; we had hauled that load on that route many times.
3" of fresh blacktop and the sign hadn't been changed.
We went through that viaduct at 20 MPH and it took the entire load of bales off of the truck.
The entire PD was called out to pick up the bales to reload the truck.
