I think it makes more sense to measure from the hitch.
Here's how I helped a friend:
We laid out the ball mount on the ground, placing it where it would give the most clearance of the camper, and retain usability of the rear step while travelling. We then measured from the hitch pin of the truck hitch to the hitch pin on the ball mount.
VOILA... we got the next size up Supertruss extension
