Try to figure out how the cushions are attached, and remove them completely. Then put a 1/4" sheet of plywood under the seat. Get exact measurements, order some foam online (lots of suppliers to custom cut to your measurements). Get a good density upholstery-grade foam.
You can make a decent inexpensive cover by cutting down a twin-size quilt, and wrapping the foam like a big gift box. Leave plenty of overlap. Pin underneath with giant safety pins. This is fairly easy to take off and wash.
Or you can spend some more $$ and get the foam professionally covered.
