I don't see the obsession with the 5-speed. Considering the modest horsepower output and lack of torque on this style engine I wouldn't expect you to be doing much towing in 5th (overdrive).
What I would worry about failing on this truck are not the big things. You're not going to see wheels or axles fall off the truck. It's going to be the little things that get you. "Old car" stuff, things you didn't update or replace when getting the truck roadworthy... Blown lines. Seals giving out. Overheating. Nothing fantastic to watch, but enough to put you on the side of the road.
