Actually, it is fairly easy to see how it happens. The dealer sells the buyer heavier bars than necessary and the new trailer owner gets obsessed with getting the front of the tv to squat back to its original height. Then some expert says "you gotta have three links hanging with that hitch". A combination of these things can cause too much force being applied to the Weight Distributing bars. In any case, a trip to the scales is the best way to get the setup right.
