The coil springs trucks have the shocks inline with the frame and the coils are on the inside of the frame rails. Notice in the picture below how the coils mount closer to the middle of the axle. The torsion bars are also closer to the middle and are angled outward to connect to the frame.

With the leaf spring suspension, the leafs are mounted inline with the frame and the shocks are on the inside of the frame. Notice how far the leafs are from the middle of the axle in the picture below. They are inline with the frame just like the shocks are on the coil springs suspension.

So lets look at the frames that are different to accommodate the two different springs configurations you've illustrated (leafs vs coils). These two pictures clearly illustrates a frame difference between a 2500 (notice frame bulges outwards to make room for the coil springs and the 3500 the frame goes straight to give room for the leaf springs.

Also compare the goose neck cross bars and how they're welded to the frame. The 3500 is straight and the 2500 has an angle do the frame differences. Also noteworthy the 2500 cross bar doubles as a goose neck mount and the coil springs upper mount.
