Under 16 proof of citizenship and if traveling with one parent a notarized statement authorizing travel may be asked for. The DUI is very problematic if criminal history is checked at border. Contact Canadian authorities before you go and receive documentation authorizing admittance into Canada, or your trip may end at the border.
An enhanced driver's license is also acceptable border documentation for land crossings. Only a few states issue them, however.
