It's late and I'm tired. So this is knee-jerk, but let me offer:
Verify that everything is GROUNDED as it should be. So often, we focus on the hot side and forget two things about ground:
1. It's always needed
2. Weak or no ground can cause feedbacks as the current looks for a path home.
