The same way the utility does for your house out on the pole except on a much smaller scale. The neutral and equipment ground are separate. In the case of this item the equipment ground(ground) from both cords are is bolted to the side of the case with a lug.

just like a normal 30 amp to 50 amp adapter with a booster in the middle. It is sharing the same 120v leg.

I cheated,

I wrote the guy and asked for a blowup of the stickers. It's nothing more than a 20 ampere rated selectable 15% boost transformer, with a switch and a pilot light.

I will take a stab at it. It looks like a common 3-5 kva 120/240v transformer rigged to to give an out put of either 120v on low or 240v on high. Either way the`watts available will be the same depending on its kva rating. Example if a 3 kva with an input of 120v the input amps will be 25 with an output voltage of 240v the output amps will be 12.5. Not sure of what benefit that would be. That being said I could see it of more benefit if it was a 12/240v 3kva used in a boost mode to increase a low voltage situation. Without a view of the label we are only guessing. My guess is that someone made this item up to fix a specific problem that they had. JMHO.

It is either a buck/boost transformer, not necessarly automatic, or a noise filter, or both.. I'm not sure it is something Id want since it has a home made look about it. Though that said, I likely could figure out how to use it to my advantage..

The LOW/HIGH switch suggests it can both boost and buck voltage.
