I have a 4" Simpson analog 0 - 150 volt AC gauge. I just now wired in a plug. The needle is dancing. It isn't an illusion. The sampling rate of digital is just too slow to catch it. Hello ballasting transformer.
Then there is the option of a Green Dot hospital power supply. They do not reduce voltage without a transformer.

Kinda sounds like it could be a dirty power issue, but I know nothing of the quality of your input power there.
I guess putting safeguards in place may be the best solution. Better to blow a cheap fuse than a phone, computer or something.

Then a 10000 mAh recharger lithium battery pack will not dispense power after using a totally different wall charger, then the final insult happened with your S4 on a third different charger. Really ****** me off.
I am using a computer for Pianotuna's S4 recharging. I wish I had access to my isolation transformers at home. Meanwhile I am going to -build- a 5 volt power supply with a one amp limit using every trick in the book. Twice in the last four months, the street lamps have gone dark, but not all at once. They are LED.
I've not audited why the cellular power management chips have failed. But in addition to the phones I have had three wall warts fail. They produce no power.
Checked line voltage two dozen or more times. 117 - 123 volts. Frequency 60.0 Hz.
This has gone beyond coincidences or miracles. But building my own 5.0 volt power supplies seems a bit ridiculous. I've never even heard or read about killing off of wall warts. And all my serious gear is 2000 miles distant.
A 1-amp fast reacting fuse will be part of the system. I did have an ANKER car charger until someone needed it more than I do. I used to charge portable stuff right off of my 12 volt Lifeline battery beside my bed then carry the power supply to the car. Back & Forth.
