My owners manual suggests leveling with slides IN.

I level before I pull out the slide. But I do keep in mind that if I am going to have our rig a bit off level left to right, I will make sure the slide side is a bit higher, not lower.

Our TT has two slides on the driver side, and one on the curb side. The kitchen slide is the heaviest and on the road side. This would change the side to side level of the trailer about 1 1/2 inches with the slides out vs with the slides in. (And of course the suspension under the trailer is designed to allow such shifts - while driving.)

After several times parking, we finally got the TT leveled exactly as we wanted. After putting the slides in and pulling up the stabilizers - I mounted a big level like you see on fivers set to indicate level, even though the rig is actually 1 inch off level.

Now when I level to that bubble - things are perfect when I put the slides out.

Always in. It would be a pita to climb under the slides, which are over the wheels, to add leveling boards. I would think it would be better for the slide mechanism to be level when extending the slides.

Level first, then slide(s)out.

Usually level up with the slides in only because I have a routine: Level side to side with blocks under wheels and set chocks, un-hitch and set end to end level with tongue jack, circle the unit setting stabilizers and locking x-chocks between wheels, then go inside.
But I also have no trouble running slides in or out in any position / conditions -- we have to do odd stuff sometimes during load-up for a trip or mid-trip stop.

Thats exactly what I do . I also complete my setting up before I lower my stabilizer jacks, this way the TT tires have had a chance to cool down.
