If the cylinders were off and you lost propane then it has to be the tanks.
BTW do you have an automatic regulator that switches between cylinders? The indicator should tell when the primary cylinder is empty. This way you fill one at a time and never run out.
Other possibility is you triggered the safety valve by opening the hand valve too fast. Were the cylinders actually empty when you went to fill them? 30lb should take about 7 gallons to fill.
