Unfortunately, I don't know the back ground, but saw this pic on another forum that was supposedly taken locally.

I looked through the forum it was posted in again. The guy that posted the picture knows the owner personally. It was NOT from a wreck, just overloading. He said they actually just straightened the frame at a collision repair place on the frame machine and called it good :E. I don't know if they welded in any reinforcements or what.
Apparently, the TC is about 4500 empty. So the truck was probably overloaded.
