We've gone through Cincinnati many times and usually took I-75 straight through. Yes, there are occasional backups, but as usual it depends on time of day. There's a lot of traffic on the KY side of the river too, but I-75 is wider there and in good shape.
What is your destination? If you're going anywhere west of I-75 (IN or western MI) I would definitely go I-275 to I-74 into IN toward Indy. If going to the east side of MI or northern OH then I-75 is best. I think I-275 is way out of the way just to avoid going through Cincinnati. Grit your teeth and do it!
